Official Rules

Send email to mirrorimagescontest@gmail.com or mail entries to P. O. Box 2676, Pine Bluff, AR 71613. Please include your Full Name, Age, City where you live and your Phone number. Please include 2 pictures: A full-on face pick and a clothed, side-body pick.

All entrants and contest winner must be 18 years or older and must, upon request, provide proof of age, be legal residents of the State of Arkansas. Only one entry per household, per person, per email address. Only one winner per household, per contest. Mirror Images and sponsor organizations are not responsible for late, lost, illegible, misdirected, misdelivered or incomplete entries. Winner will be considered from all eligible entries. All entries must be emailed to mirrorimagescontest@gmail.com by 12/1/09.

Prizes Prize will vary in value depending services utilized. Taxes are the responsibility of contest winner.

How to Enter

Sponsor(s) will not be responsible for lost, late, forged, mass-reproduced or mass-transmitted, misdirected, mutilated, incomplete or illegible entries and such entries are void; telephone, electronic, hardware or software program, network, Internet or computer malfunctions, failures or difficulties, errors or failures in transmission or reception and/or any condition caused by events beyond the reasonable control of Company which may cause the contest to be disrupted or corrupted. Odds of winning Prize depend on the number of eligible entries. Decisions of the judges are final. There are no prize or cash substitutions. Employees of Contest Sponsors and their immediate families are ineligible to participate in this Contest. This Contest is void where prohibited by law. This Contest is subject to all federal, state and local laws.

Winners

Winner will be considered from all eligible entries. The decision of the judges is final. Winner will also be notified by phone. Winner must acknowledge the notification by phone call made to Company no later than 2 business days after the judges decision or the prize will be forfeited. Company is not responsible for late, misdirected, or unsuccessful efforts to notify Contest winner or to acknowledge receipt of notification.

(1) Contest is open to all persons, regardless of age, gender or race. Contest winner is subject to limitations based on medical restrictions. (2) If selected, the contest winner understands that treatment will occur in the office of Mirror Images in Pine Bluff, AR under the care and supervision of Dr. Anna Redman and Dr. Nancy Williams. (3) Contest winner reserves the right to decline treatment(s) recommended by the physicians at Mirror Images, however such refusal disqualifies them as the winner, and the contest prize will be awarded to the runner up. (4) Contest winner agrees to permit photo and/or video documentation of their beauty journey. (5) All decisions made by the judges are final. (6) Winner understands that results cannot be guaranteed

Contest winner will be required to execute a declaration of eligibility that winner has complied with all the rules and release Sponsor(s) of all liability in connection with the prize’s goods or services, the winner releases)Sponsor(s) and all prize-supplier companies from all liability for damages or personal injury in connection with the winner's use of the prize goods or services and consent that the Sponsor(s) and anyone it/they may authorize may without compensation use winner's name, photograph or other likeness, biographical information and statements concerning the Contest or the Sponsor(s) for purposes of advertising and promotion. Odds of winning are determined by the total number of correctly answered entries received. Taxes are the responsibility of Contest winner. Company reserves the right to cancel or modify the contest.

Qualification

The Contest is open to legal residents of Arkansas who are 18 years of age or older. Employees (including immediate family members) of Mirror Images, and their affiliates, subsidiaries and advertising agencies, are ineligible to participate or win.

All entries become the sole property of Mirror Images and will not be returned. By entering, entrants agree to abide by these rules, and warrant and represent that their entry is their original work, and grant Mirror Images the right to edit, publish, promote and otherwise use their entries without further permission, notice or compensation.

Compliance with Law

Void where prohibited by law. All federal, state and local laws and regulations apply. Payments of all federal, state and local taxes are the sole responsibility of the winner and winners may receive an IRS Form 1099 or equivalent from Mirror Images.
